Job summary
This role is set in a but acute specialist nursing service, where the team manage and optimise care for people living with diabetes.
This post is for a band 7 nurse to support the nurse led diabetes virtual ward and work alongside the inpatient team. You will need to have a good knowledge and confidence to triage and assess patients remotely. This is innovative service and we are excited to be able to grow the team and the service further. You will also be responsible in supporting the small diabetes virtual ward team and work alongside the Advanced Clinical Practitioner.
The post holder will be a key resource for other medical and nursing staff within the trust, contributing to patient reviews and policy reviews and developing the existing service.
There will be opportunities to work alongside clinicians who work in the renowned Leicester Diabetes Centre which conducts first class clinical research in diabetes.
Main duties of the job
You will be providing a comprehensive nurse led service for patients who need urgent support to enable them to either be discharged from hospital earlier or prevent them from being admitted. Part of the role will be opportunistic education with patients to empower them to self-manage their diabetes. IT skills are important in this role.
Working within the multidisciplinary team, you will support medication concordance and encourage optimisation with prescribed treatments. Educating and supporting patients and their families/carers, along with healthcare professionals with your specialist knowledge, you will encourage best practice to achieve best health outcomes for patients.

Ensuring good quality consistent care for people with diabetes and to enhance the care of patients with diabetes across all pathways.
