Leicester and Nottingham university Hospitals have been awarded additional NHS funding to further develop our Paediatric Rheumatology service. As a result an exciting opportunity has arisen to join our growing Paediatric Rheumatology service as a band 3 support worker. You will be working alongside a dedicated team of consultants, Specialist Nurses and allied health professionals. you will provide support to the clinical team and act as a support and resource for our service users
Main duties of the job
To work as a member of a team providing a high-qualityservice to children and young people within PaediatricRheumatology keeping in close contact with patients, theirfamilies and others involved in the day-to-day care. Offersupport and advice (under supervision) to optimise themanagement of patient's conditions and their physical,psychological, intellectual, educational, and socialdevelopment.To assist the Rheumatology team in fulfilling the day-to-daylogistics of the service; clerical support, follow up ofpatients by phone/text /email contact and updating ofdatabases.Assist in developing health promotion and education inconjunction with other health care professionals ensuringthat all patient care within the multi-disciplinary team isbased on research.Be an accessible, approachable member of the team as afirst point of contact for patient and families.

To liaise with families in person, by telephone, email and in writing as appropriate in relation to aspects of care, which have been pre determined. To maintain accurate record-keeping on databases and in medical notes of liaison with families; patients; schools or other childrens care providers and Primary Care Teams. To undertake home visits when required to support families. To organise equipment required at home for families. To take an active role in supporting families face to face and with the use of technologies. To organise and display patient information and health promotion boards Arrange and obtain blood samples when required To undertake community/ home visits as required either on their own or jointly alongside the team to cover both schools teaching to staff and / or patients in their home / school as needed. To refer any patient concerns to the Paediatric Rheumatology team. Act as the patients advocate ensuring that their best interests are met. Have a thorough and up to date understanding of safeguarding children and young people. Undertake clinical observations and procedures (weight/ height, blood pressure, and urinalysis) within theoutpatient setting, and report appropriately. Support the organising of patient investigations and monitoring, reminding families to attend. Communicate with and provide support for children, young people and their families and other agencies such as education and social care. To access charities to support families as appropriate. Actively participate in the Paediatric Rheumatology Nurse Network To collate the information required for new patients as identified by the team. To ensure that new patients are provided with relevant information To print/post information to families as required. Order and organise patient information Organise appointments for children requiring day-care treatment To maintain accurate records of all contacts made with the child and family), which should be available to colleagues, senior nursing/medical staff. Records should reflect the present condition and progress of the child. To assist with audits, research and patient satisfaction questionnaires identified by the Paediatric Rheumatology Team. Be actively involved in data collection for the service alongside the lead Nurse to support the development of the Paediatric Rheumatology service. To carry out photocopying, make telephone calls and take messages as required. To help organise training sessions. Accurately record patient activity on currently used systems (i.e. Patient Centre) Complete patient information forms required for Homecare companies.Governance Undertake only those duties and delegated tasks and treatments within own limitations, level of competence, skill and training. Assist in the delivery of direct and indirect care within Directorate, Trust, local and nationally agreed protocols and policies. Assist with the collection of simple data for audit/research and other purposes as delegated by the Registered Nurse/Health Care Professional/Multi-Disciplinary Team. Contribute to the management of risk through assessment processes to ensure that risks are reduced at all times to the lowest minimal levels. Adheres to Trust policies, procedures and guidelines and promotes the compliance of others.Patient/Customer Service Promote the philosophy of patient centred care and establish and maintain a supportive relationship with the patient and their family at all times. Promote patient and public involvement and enhancing their experienceby supporting and implementing new initiatives relating to patient centred care.
