This is an exciting secondment opportunity for a physiotherapist to lead the neuro-rehabilitation outreach therapy team until the end of April 2027.
The Community Neuro-rehabilitation Outreach team provide a comprehensive and co-ordinated rehabilitation service to people across Lincolnshire with a long term neurological condition or following multiple trauma who have complex rehabilitation needs. The team includes occupational therapists, physiotherapists, clinical nurse specialists and a dietitian and has close links with the Rehabilitation Medicine consultants, Ashby Ward and the wider health and social care system across Lincolnshire. The service provides rehabilitation to patients at home and in outpatient settings and covers the whole of Lincolnshire with staff based at 4 sites: Louth, Boston, Lincoln and Grantham. Regular travel to each of these bases is required as part of this role.

Main duties of the job
To lead the delivery of personalised, holistic neuro-rehabilitation outreach services ensuring good communication links with the Clinical Nurse Specialists, Rehabilitation Medicine Consultants, Ashby ward team, Outpatient Therapists & Site Leads. The post holder will work closely with health, social care and voluntary sector partners across neighbourhoods to achieve best clinical outcomes and recovery for patients
- To support the team in delivering a high quality, safe, cost effective, patient focused service, with effective demand management systems which are responsive to the changing local need utilising technological options to maximise efficiency and safety.
- To work closely with Ashby Rehabilitation unit Clinical Nurse Specialist/Discharge Coordinator to ensure effective patient flow along the Rehabilitation pathway and seamless transition into the community.
- To provide support and guidance to inpatient therapy staff for patients known to the Neuro-Rehabilitation Outreach team in order to ensure there seamless transition back into the community.
- Provision of day to day management and ensuring appropriate levels of supervision and appraisal are in place for staff within the clinical team.
- Maintain a clinical caseload, with clinical activity accounting for between 60-75% of working hours
