Due to significant investment we are looking for experienced children's nurses to join our Lincoln CCN Team. If you are passionate about community nursing and want to be part of our exciting journey - we would love to hear from you! You will have responsibility of managing your own caseload and coordinating services to facilitate early discharge, reduce hospital re-admissions and provide support and nursing care to children and young people with acute healthcare needs and those with long-term conditions at home. Participating in the out-of-hours telephone on-call system to support all children and families on the CCN countywide caseload you will enable admission avoidance.
As an excellent communicator you will support the development of ambulatory care and promote the ethos of empowering children and their families to develop the knowledge, skills and expertise to enable them to become self-caring, independent and manage their own condition at home wherever possible.

Opportunities for professional development, training and regular 1:1 support from the Team Leader will be offered to all successful applicants.
