The post holder will carry continuing responsibility for a defined caseload, including the assessment of care needs and the development, implementation and evaluation of complex care programmes, ensuring continuous provision of integrated care.

To provide on-going holistic care within the community setting for vulnerable neonates and their families.

To provide specialist on-going education, advice and support to the family in the home environment.

To take a proactive role in the education and training of the multidisciplinary team(s), providing on-going supervision and support as an expert resource for all disciplines where neonates require on-going care needs within neonatal outreach.

Main duties of the job

United Lincolnshire Hospitals NHS Trust is currently seeking dynamic, enthusiastic and motivated experienced Neonatal Nurses, to help with the implementation of a new Neonatal Outreach service. Neonatal Outreach will contribute to the creation of network-wide homecare provision within the East Midlands region.

Band 6 Neonatal Nurses based at ULHT are required to promote safe early discharge of babies from the neonatal units at both Lincoln and Boston. Families will be supported to provide ongoing care needs in the home environment, for example the provision of nasogastric tube feeding at home.

This will not constitute a formal merger of the two organisations, but will bring the Trusts together under a single Board and Executive Leadership Team, with the goal of improving the care that is provided to patients both in the community and in hospitals across Lincolnshire. Both Trusts will retain their separate statutory names and legal obligations.

The Group will be known as Lincolnshire Community and Hospitals NHS Group (LCHG).

  • Current First Level Registered Nurse/RSCN or equivalent.
  • NMC registered
  • Neonatal Intensive Care post registration qualification..
  • Evidence of recent professional development.
  • Hold current NLS provider certificate
  • Evidence of a good command of written and spoken English
  • Degree or willingness to work towards

Desirable

  • Community care of the neonate qualification

Previous Experience

Essential

  • Recent experience in Neonatal nursing at Band 5/6
  • The ability to work as part of a multi-disciplinary team.
  • Ability to question current practice
  • Demonstrate ability to influence and to motivate the clinical practice of others
  • Committed to on-going professional development

Knowledge/skills/aptitudes

Essential

  • Good written and oral communication skills when liaising with colleagues, patients, families, carers and the multi-disciplinary team
