Job summary
The post provides a home enteral nutrition and nutritional support service in the community to some of our most vulnerable patients. It also covers general outpatient clinics to a range of patients requiring dietetic support, conducted via telephone, video and face to face.
The post is suitable to dietitians with only minimal post-graduate experience as you will be given the opportunity to gain relevant experience and develop your knowledge and skills in community services and home enteral nutrition. You will be part of a friendly and dedicated team of dietitians and dietetic assistants, whose aim is to provide an excellent, compassionate and cost effective service. The post provides also provides home visits to those patients unable to attend a hospital appointment in addition to community based clinics.

Main duties of the job
To provide a specialist, cost efficient and high quality nutrition and dietetic service to support patients primarily requiring enteral tube feeding and nutritional support in community settings.
To monitor and care for a dynamic caseload of patients, some with complex nutritional needs
To provide this service to clients and carers in community clinics, nursing/care homes and patients' own homes via telephone, video consultation and home visits as required.
To contribute to evidence-based policy, guidelines and procedures in managing this client group.
To reduce the risk of malnutrition in this client group, both by individual consultation and education to carers and staff in community settings.
To support appropriate and cost effective use of enteral feed and enteral feeding equipment, and of oral nutritional supplements (ONS) in the community.
To work with other agencies and healthcare professionals to promote the organisation of good and effective nutritional support in community settings, primarily care/nursing homes.
To provide dietetic cover for other defined areas.
To contribute to the development and evaluation of the post.
To be compliant with national and local standards related to nutritional support.
