Our Acute Integrated Frailty Service has an exciting opportunity for a Frailty Advanced Clinical Practitioner - Band 8a
Main duties of the job
The successful candidate must be; a qualified ACP not necessarily in Frailty as support will be provided to work through the Frailty Competencies , be flexible, innovative, highly motivated and above all passionate about improving the care and assessment of Frail older people in Lincolnshire.
Location: Lincoln County Hospital , but may be expected to be able to work across all sites within ULHT to meet the needs of the Frailty Service. There may also be a requirement to work as needed across partner organisations as part of the vision for an Integrated Frailty Workforce for Lincolnshire.

-The service will run 7 days a week with hours eventually extending from 08:00 - 20:00

Successful candidates will work as part of our Front Door Service providing clinical expertise in A&E, our Medical Admissions Unit and in our Frailty SDEC. Successful candidates will also input into the county wide Frailty Virtual Ward. Successful candidates will strive to prevent unnecessary A&E attendances, hospital admissions and the care closer to home principles

- Current Professional registration within area of practice
- Post registration qualification or training relevant to speciality
- MSC Advanced Clinical Practice
- Registered Independent Non-Medical Prescriber
- First Degree
- Evidence of teaching, supervision and assessment skills
Desirable
- MSc in Frailty
- Older Adults Fellowship (HEE)
- Mentor/assessor qualification
Previous Experience
Essential
- Substantial post registration relevant experience
Desirable
- Management and leadership experience
- Completed portfolio of supervised practice in Frailty
