Job summary

Our Acute Integrated Frailty Service has an exciting opportunity for a Frailty Advanced Clinical Practitioner - Band 8a

Main duties of the job

The successful candidate must be; a qualified ACP not necessarily in Frailty as support will be provided to work through the Frailty Competencies , be flexible, innovative, highly motivated and above all passionate about improving the care and assessment of Frail older people in Lincolnshire.

Location: Lincoln County Hospital , but may be expected to be able to work across all sites within ULHT to meet the needs of the Frailty Service. There may also be a requirement to work as needed across partner organisations as part of the vision for an Integrated Frailty Workforce for Lincolnshire.

- 30hrs

-The service will run 7 days a week with hours eventually extending from 08:00 - 20:00

This is a fantastic opportunity to be a part of our dynamically evolving Integrated Frailty Service. Successful candidates will work as part of our Front Door Service providing clinical expertise in A&E, our Medical Admissions Unit and in our Frailty SDEC. Successful candidates will also input into the county wide Frailty Virtual Ward. Successful candidates will strive to prevent unnecessary A&E attendances, hospital admissions and the care closer to home principles

  • The ACP will be accountable for independent clinical assessment, diagnosis and treatment of patients who are frail as part of the Acute Integrated Frailty Service.

Qualifications

Essential

  • Current Professional registration within area of practice
  • Post registration qualification or training relevant to speciality
  • MSC Advanced Clinical Practice
  • Registered Independent Non-Medical Prescriber
  • First Degree
  • Evidence of teaching, supervision and assessment skills

Desirable

  • MSc in Frailty
  • Older Adults Fellowship (HEE)
  • Mentor/assessor qualification

Previous Experience

Essential

  • Substantial post registration relevant experience

Desirable

  • Management and leadership experience
  • Completed portfolio of supervised practice in Frailty

Evidence of Particular Knowledge, skills and Aptitudes

  • Ability to evidence / demonstrate key values and behaviours in line with Trust Framework
  • Able to present and receive complex information to multi-professional team
