We are looking for two motivated pharmacists to lead an innovative new cross-sector service in Boston, Lincolnshire.
Rural Division of First Coastal Primary Care Network (RDFC) and United Lincolnshire Hospitals NHS Trust (ULHT) are working together to address systemic challenges relating to continuity of care for patients across healthcare sectors. The aims of the service are to bridge the gap between primary and secondary care, to proactively manage patients' needs and to improve experience and outcomes for those members of our community that are most at risk.
This is an excellent opportunity for a pharmacist who wants to develop their leadership skills and work as a part of a multidisciplinary team across organisations.

Main duties of the job
Based at Pilgrim Hospital and within RDFC surgeries, you will be expected to facilitate the safe discharge and ongoing review for high-risk patients. Patient focus will be based on system level challenges and therefore the role may include working alongside our ICS colleagues.
The role aims to improve patient care by:
- Bridging communications between primary and secondary care and reducing fragmentation
- Shared decision making and joint care with multidisciplinary teams (MDT) across primary and secondary care
- Involving patients in decisions relating to their care
Specifically by:
- Reconciliation of medicines at discharge within hospital and in GP surgeries
- Reducing polypharmacy and optimising the use of medicines
- Providing pharmacological support to the MDT
- Increasing clinical interventions
- Improving compliance
- Reducing medication errors
- Improving the discharge process and ongoing care needs
- Providing a structured review process
This role would be ideal for someone that has a passion for challenging practices and procedures to drive service development.
