Job summary

Are you an experienced and accomplished registered children's nurse looking for a new challenge? Do you want to make a difference to the lives of children and young people 13-25 years who may have a long-term condition, be technology dependent and / or have complex medical needs ? If this is you, then read on! We have an amazing opportunity within our community children's nursing service to join us as a Roald Dahl Transition Nurse Specialist for this cohort of children and young people.

This new and challenging post will be responsible for developing a seamless service Monday-Friday 9.00am to 5.00pm to a caseload of children and young people and their families at the beginning and throughout their transition journey moving from children's to adult services.

Applicants must be NMC RN (Child). Recent experience of working within children's nursing, including care of children with long-term conditions and / or complex medical needs is essential. An education to masters level and / or community specialist practitioner qualification / experience is both desirable and advantageous.

Main duties of the job

You will be part of the Family Health Division's Transition Core Working Group ; working closely with the children's and adult consultant transition leads, children's community matron, transition project manager and partner organisations, developing and improving transition care pathways.

As a member of Lincolnshire's Transition Network, you will work collaboratively across multi-agency / organisational boundaries providing expert advice to promote high quality transition planning and joint working. Working closely with local paediatric and adult hospital services, departments and teams and those at tertiary centres, you will also liaise with professionals from across the system eg primary care, health and social care services, education and voluntary services.

In partnership with the child / young person and their family and all paediatric and adult professionals involved in their care, you will formulate a personalised holistic transition plan incorporating the child / young persons feelings, wishes and goals (where possible)
