Job summary

The Progress Chaser is responsible for monitoring, tracking, and actively following patient pathways and administrative processes to ensure timely progression through services. The role supports the delivery of safe, effective, and efficient patient care by reducing delays, maintaining accurate records, and escalating issues appropriately.

Main duties of the job

The Progress Chaser is responsible for supporting the effective management of patient pathways to ensure timely progression of care in line with NHS targets and Trust performance standards. The post holder will proactively monitor patient journeys, identify delays, and follow up on outstanding actions such as appointments, test results, and clinical documentation. Working closely with clinical and administrative teams, the Progress Chaser helps maintain accurate patient records, supports waiting list management, and escalates risks where necessary to prevent breaches of Referral to Treatment (RTT) standards. A key element of the role involves ensuring timely referrals to community services and multidisciplinary inpatient teams, facilitating safe discharge planning and continuity of care. The post holder will liaise closely with clinical staff, discharge coordinators, community providers, and MDT members to prevent delays and support smooth transitions between services. The role will also include supporting patients with basic care needs, assisting with patient flow, and providing reassurance and practical support to enhance the patient experience.
