Children's Diabetes Specialist Nurse

Job summary

The post holder will work as a member of the Diabetes Nursing Team under the direction of the Consultants to deliver an expert nursing service to patients with Diabetes, from diagnosis until discharge or transition to the adult service. The post holder will deliver specialist nursing skills, demonstrate sound clinical knowledge and expertise in the autonomous management of children and young people within the diabetes service. The post holder will ensure that patients and families are educated, informed and supported by providing clinical and social support in relation to their diagnosis. It will be necessary to work flexibly with a caseload of patients, across both the hospital and community settings for a defined caseload of patients within the diabetes service and to provide symptom advice to other health care professionals and initiate guideline-based therapies. The post holder will establish clear lines of communication with primary healthcare teams . The post holder will contribute to the development of evidence based clinical nursing practice in line with current research and guidelines and will participate in audit and quality improvement initiatives.

The post holder will ensure that patients and their families are educated, informed and supported by providing clinical and social support in relation to their specific diagnosis and treatment plans

Main duties of the job

  • It will be necessary to work flexibly across a caseload of patients across both the hospital and community setting for local patients and to provide expert advice to other health care professionals and initiate guidelines based therapies that address acute management issues for diabetes
  • The post holder will contribute to the development of evidence based clinical nursing practice in line with current research and guidelines and will participate in audit.
  • The post holder will work across organisational, regional and national boundaries and form good working relationships with colleagues within the regional diabetes network.
