Administrator Discharge Liaison Team

Job summary

As an administrator within the Discharge Liaison Team the post holder will be responsible for all aspects of secretarial support.

In addition the post holder will be expected to work with minimal supervision and maintain and update the Discharge Liaison website. The post holder will be expected to advise nursing staff and offer support in the use of the electronic tool for document submission as required.

The post holder will be expected to ensure that the delayed discharge list and SITREPS are available when required and run ad hoc reports regarding discharge as required.

Main duties of the job

Plans and carries out work liaising with colleagues.

Provides an efficient service welcoming visitors, dealing with enquiries as first point of contact in person or by telephone

Communicates confidential and sensitive information to other Trust staff and outside agencies.

Uses tact and experience when taking telephone calls from patients/carers who may be feeling upset or anxious.

Understands and clarifies detailed instructions to undertake tasks.

Ensures effective communication is maintained between members of the team taking personal responsibility to update and share information.

Uses appropriate communication methods and takes steps to modify if communication barriers are created.

Produces accurate written information and statistics in a timely manner as requested.

Ensures that electronic communication is accurate and is delivered in a timely manner.

Is able to communicate with ward staff and external agencies e.g. Social Services, members of the PCT's, to ensure that the electronic process is used appropriately.
