Job summary
The Waiting Well Coordinator will actively manage and support patients while they are on the waiting list for orthopaedic operations, mainly hip and knee replacement procedures, whilst ensuring a smooth and efficient service for both the multidisciplinary team and patients, in order to deliver a high quality patient focussed service.
The postholder will have a crucial role in supporting adult patients awaiting surgery, enabling them to identify and address their health and wellbeing needs and being a knowledgeable point of contact
The post will require the successful applicant to use information provided by the patient to sign post the patient to the appropriate pathway to optimize them prior to their surgery.
The WWC will work closely with the trust Electronic Patient Record 'Lorenzo'
When the patient is seen by the consultant in the outpatient clinic and placed on the waiting list for surgery, a number of modifiable risk factors will be listed on an 'add to waiting list form' on Lorenzo. The patient will also have blood tests when they are listed for surgery and will be registered on the relevant PKB care plan.
Please note that this advertised vacancy does not meet the UKVI eligibility requirements for a Skilled Worker Visa and therefore HUTH would not be able to issue a Certificate of Sponsorship for this role
Main duties of the job
The Waiting Well Coordinator will appraise the 'add to waiting list' form to identify the modifiable risk factors relevant and contact the patient to signpost them to the appropriate service

These resources already exist and the WWC will encourage the patient to ring or email the relevant group.
The WWC will keep a record of which pathways the patient has been referred to and will contact the patient again after 2-3 months to ensure they are completing the relevant health optimisation
The role is for two days a week on a 6 month basis, to establish the links with both Outpatients, surgical listings and pre-assessment to develop access points earlier in the pathway. Then a selection of patients would be referred and managed with by the WWC linking with appropriate Trust and partner organisations. This will include referrals for the Goole HUB.
This will run for 6 months with the potential to extend to 12 months, to provide a cohort to assess against the non-optimised patients and review;-
