Memory Assessment Nurse - Permanent, part-time (2 days a week)

We have a great opportunity for a registered mental health nurse, working across the High Weald, Lewes and Havens area of East Sussex for our Memory Assessment Service. The post will be based at Hill Rise in Newhaven which is a fishing port surrounded by the South Downs National Park and less than ten miles from the vibrant city of Brighton.

Main duties of the job

The Memory Assessment Service is a relatively small nurse led team that provides assessment, diagnosis and treatment initiation for Dementia. Referrals all come from GPs. Following Triage screening, the Memory Assessment nurses meet with patients and often their carers, to conduct comprehensive assessments which are then discussed with our Dementia specialist Consultant and our Non-medical prescribing Senior Nurse practitioners then deliver the diagnosis and commence prescribing if indicated. Patients are generally discharged back to primary care post diagnosis or their care is transferred to the Specialist Older Adult team for follow-up.

They will demonstrate an understanding of clinical interventions relevant the care group to include education, care plans and discharge planning, working closely with carers, families and partner agencies to facilitate this. The post holder will work at all times to promote the safety and the well being of service users and their families/carers. The post holder will assess, plan and implement care, and provide specialist nursing advice and carry out specialist nursing procedures. They will provide clinical supervision to staff and students and acting as a mentor for student nurses.

  • Experience of partnership working within and across various statutory and non statutory teams and agencies
  • Experience of working within the CPA process
