Job summary

We have a community nursing opportunity within ourIntensive Support Team (IST), Older Adults Mental Health service within Wealden Neighbourhood Mental Health Team based at Uckfield. The post will suit a Registered Mental Health Nurse but will consider Learning Difficulties Nurses who have sufficient experience of working with Dementia or severe mental illness and alsoBand 5 Nurses willing to do Band 5 to 6 Mental Health Nurse development post.

The post involves working with older people with complex mental health and physical health care needs. You will be expected to support both the client and their carers. You will also need to be able to work cohesively with other services within our Trust and with other partner agencies.

You will be caring, compassionate, a good communicator, with a commitment to improving the lives of our service users through creative, evidence based practice. You will be working collaboratively with service users and their carers on the IST caseload to create care plans that meet their needs and maximise their independence.

There will be travel across the geographical area covered by the team, the ability to travel to remote areas is essential.

The post is 37.5 hours per week withshift pattern, including extended hours and weekend working.

Main duties of the job

The post holder will be an autonomous practitioner who will carry continuing responsibility for a defined caseload of service users by undertaking the following duties:

  • To undertake assessments of individuals and their families in the designated care group with complex health presentations, including those service users presenting with higher levels of risk.
  • To develop alongside the service user their care plan.
  • To offer specialist nursing assessment and advice where required.
  • To assess and manage on going risks as identified during the assessment, ensuring that this is done in collaboration with the service user and carers. To regularly review risk factors and make changes to the management of them as necessary.
  • To provide supervision of junior staff and trainees where appropriate.
