An exciting opportunity has arisen to join the bank for the East Sussex Rehabilitation Pathway.

The East Sussex Rehabilitation Pathway provides specialist community and inpatient mental health rehab services for adults over the age of 18, living within East Sussex. We believe in providing outstanding quality, safe and effective care and treatment that everyone can be proud of.

The ward works closely with Community Rehabilitation colleagues as an integral part of theaward-winning East Sussex Rehabilitation Pathway.

Main duties of the job

The purpose of this post is to work as a member of a team which provides mental health services, and to focus on the day to day support needs of service users. The post holder will provide support to an allocated group of service users with more complex needs, promote recovery and maintain their independence within the community.

The post holder will work with other members of the multidisciplinary team to facilitate the development and delivery of community care. This includes contributing to the full range of care tasks, assisting the care co-ordinator to assess and plan, implement and evaluate individual care plans, co-ordinating work under supervision, taking on the role of care co-ordinator under supervision, co-facilitating therapeutic groups and individual activities, information gathering and analysis to inform a multi-disciplinary formulation, audit and specific aspects of therapeutic intervention.

Job responsibilities

  • Work with the multi-disciplinary team to establish and deliver a c programme of care for patients on a daily
  • Undertake a range of delegated clinical and administrative duties.
  • Contribute to a full range of routine care tasks such as supporting self-care, routine observations and risk assessment once trained and under supervision from a registered professional
  • Provide and facilitate recreational, social and educational group and individual activities as part of the Ward / hospital wide in-patient programme; following set aims and objectives
  • Co-facilitate therapeutic groups under the supervision of a registered professional appropriate for the service user group
  • Engage with and gather information from service users, relatives and patient records to support a multi-disciplinary formulation and care plan

Provide assistance and support with welfare benefits and patients financial situations.

Undertake venepunture and ECGs as trained.

  • Deliver specified aspects of therapeutic interventions, once trained, under close supervision from a registered professional.
  • To ensure all service users are aware of activities which may be of benefit to them and how these can be accessed.
  • To contribute to assessment which will include assessment tools such as venepuncture, ECG and cognitive testing and support the development and implementation of recovery plans.
  • Following an assessment, to assist in interventions with service users implementing the recovery plan that has been agreed. This may involve working with groups or individuals.
  • To initiate, develop and implement new activities in response to service users needs, where appropriate following evidence based practice.
  • With the multi-disciplinary team contribute to the relevant risk assessment.
  • To ensure therapeutic areas and appropriate materials are prepared for use in the activities, monitoring potential risk.
  • To be responsive to service users needs and choices; and uphold their right to be treated with dignity and respect.
