Job summary
We have a great community nursing opportunity within our Intensive Support Team (IST) at Weald Neighbourhood Mental Health Community Team, Specialist Older Adults Mental Health service based at Millwood in Uckfield, East Sussex. This post will suit a Registered Mental Health Nurse, but we will consider Learning Difficulties nurses who have sufficient experience of working with Dementia or Severe Mental illness and also Band 5 Nurses willing to do Band 5 to 6 Community Mental Health Nurse development post.
You will be a key part of our multi-disciplinary service for older adults with mental health problems, including Dementia. This is challenging work but we receive very positive feedback from our service users and carers.This post will involve working with older people with complex mental health needs as well as physical health care needs. You will be expected to support both the client and their carers. You will also need to be able to work cohesively with other services both within our Trust, and with other partner agencies.
You will need to be caring, compassionate and a good communicator, with a commitment to improving the lives of our service users through creative, evidence based practice. As a band 6 nurse you will be responsible for working collaboratively with service users and their carers on the IST caseload to create care plans that meet their needs and maximise their independence.
The post based at Millwood, Uckfield and is 22.5 hours per week,
Main duties of the job
The post holder will be an autonomous practitioner who will carry continuing responsibility for a defined caseload of service users by undertaking the following duties:
- To undertake assessments of individuals and their families in the designated care group with complex health presentations, including those service users presenting with higherlevels of risk. To develop alongside the service user their care plan.
- To offer specialist nursing assessment and advice where required.
- To assess and manage on going risks as identified during the assessment, ensuring that this is done in collaboration with the service user and carers.
- To regularly review risk factors and make changes to the management of them as necessary.
- To provide supervision of junior staff and trainees where appropriate.
