Carer STR Worker - Specialist Older Adults Mental Health Service

If so, our Band 4 Support Time Recovery Worker role in our Specialist Older Adults' Mental Health Service (SOAMHS) may be the role for you!

We are seeking to recruit a highly motivated and skilled Band 4 STR worker. This post is for 37.5 hours per week to work with our enhanced duty team. This team is part of SOAMHS and provides intensive support to older people with dementia or mental ill health. The post holder will work on a rolling rota and the hours will be 9-7 which aligns to the working hours of the Intensive Support Team.

Main duties of the job

-To provide intensive support to older adults and people living with dementia.

-To reduce hospital admissions and mental health act detentions. This includes dementia and mental health inpatient admissions.

-To work in collaboration with families and carers and support them with their caring role

-To reduce carer breakdown due to strain of caring role

-To work alongside the multidisciplinary staff in the Intensive Support Team

-To provide personalized care and support to older people and their family / carers. Support with the development of contingency and crisis care planning.

-Signpost and support family / carer to access community and social activities to reduce carer loneliness and isolation. Work alongside the Soamhs Carer peer support worker to identify appropriate VSCE who can support in line with the ethos of the Neighbourhood Mental Health Teams.

-Contribute to formulation and multi-disciplinary team meetings

-There will be an expectation that the carer support worker will have capacity to spend extended periods of time with the person in crisis and their family / carers. Advise on psychosocial interventions which may reduce distress for both the patient and their family / carers.
