Job summary

We have a great community nursing opportunity within the Weald Specialist Older Adults Mental Health service based at Millwood in Uckfield, East Sussex. This post will suit a Registered Mental Health Nurse but will consider Learning Difficulties Nurses who has sufficient experience of working with Dementia or Severe Mental illness and also Band 5 Nurses willing to do Band 5 to 6 Community Mental Health Nurse development post.

This post will be a key part of our multi-disciplinary service working with older people with complex mental health needs, including Dementia as well as physical health care needs. You will be expected to support both the client and their carers. You will also need to be able to work cohesively with other services both within our Trust, and with other partner agencies.

You will need to be caring, compassionate and a good communicator, with a commitment to improving the lives of our service users through creative, evidence based practice. As a Band 6 nurse you will be responsible for working collaboratively with service users and their carers to create care plans that meet their needs and maximise their independence.

There will be travel across the geographical area covered by the team, the ability to travel to remote areas is essential.

We will support the transition from Band 5 to Band 6.

The post 37.5 hours per week, Monday to Friday 9.00 am to 5.00 pm (excluding Bank Holidays)

Main duties of the job

The post holder will be an autonomous practitioner who will carry continuing responsibility for a defined caseload of service users by undertaking the following duties:

  • To undertake assessments of individuals and their families in the designated care groupwith complex health presentations, including those service users presenting with higher levels of risk.
  • To develop alongside the service user their care plan.
  • To offer specialist nursing assessment and advice where required.
  • To assess and manage on going risks as identified during the assessment, ensuring thatthis is done in collaboration with the service user and carers.
  • To regularly review risk factors and make changes to the management of them as necessary.
  • To provide supervision of junior staff and trainees where appropriate.
