The East Sussex Rehabilitation Pathway provides specialist community and inpatient mental health rehab services for adults over the age of 18, living within East Sussex. We believe in providing outstanding quality, safe and effective care and treatment that everyone can be proud of.

The ward works closely with Community Rehabilitation colleagues as an integral part of theaward-winning East Sussex Rehabilitation Pathway.

In this rewarding role, you'll support individuals living with severe and enduring mental health conditions, guiding them through a recovery-focused rehabilitation model. You will hold a caseload of individuals with complex mental health needs, helping them to develop positive coping strategies that foster independence and personal growth.

Working collaboratively with a diverse multidisciplinary team, including psychologists, psychiatrists, occupational therapists, support workers, and third-sector partners, you will play a key role in providing holistic, person-centred care.

Main duties of the job

The role of Community Nurse is to work in partnership with service users in the designated care group with complex health presentations, developing their care plans and recovery plans to enable them to lead fulfilling lives. They will demonstrate an understanding of clinicalinterventions relevant the care group to include education, care plans and discharge planning,working closely with carers, families and partner agencies to facilitate this. The post holder will work at all times to promote the safety and the well being of service users and theirfamilies/carers. The post holder will assess, plan and implement care, and provide specialist nursing advice and carry out specialist nursing procedures. They will provide clinical supervision to staff and students and act as a mentor for student nurses.
