We are looking for an experienced and passionate Team Leader to take the helm of a high-performing Crisis team based in Chichester, driving excellence in patient care and service delivery. Working alongside the Clinical Nurse Specialist and under the leadership of the Service Manager, the Team Leader will play a pivotal role in ensuring the Crisis Team delivers holistic, patient-centred, and safe care across the Urgent Care Pathway. You will work closely with the Mental Health Liaison Team at St Richards Hospital, Chichester and The Haven at Meadowfield Hospital to provide seamless support to those in crisis.
The CRHTT has a Discharge Team Lead that works closely with patients who live in Chichester and Bognorare in inpatient care and completes Early Assessments as part of the CRHTT.
