Job summary

As part of our East Sussex community transformation plans we are starting to develop our Neighbourhood Mental Health Teams. For Specialist Older Adult services there will be 4 teams across East Sussex with a range of multi disciplinary professionals in each team who will provide assessment, treatment and interventions including the provision of intensive support and treatment to enable people to receive care and treatment in their home setting and avoid the need for hospital admissions

Main duties of the job

We have Band 4 roles, for Support, Time & Recovery Workers ( STR Worker), who will provide intensive support and treatment to people in the community. These post holders will work alongside nurses and OT's and will be providing intensive support and interventions to patients in their own homes preventing them from needing an admission to hospital. We would like you to join us and be at the forefront of leading the community transformation changes.
