We are looking for a Mental Health Practitioner to join our fantastic team.
The Hampshire CAMHS Countywide Eating Disorders Team has a vacancy for a Band 6 Mental Health Practitioner.
We are a diverse team and strive to provide high quality, evidence based compassionate care to young people and families. The teams therapeutic model is based on systemic theories and practice which enhances the family based treatment provided.
We are looking for a practitioner that shares similar values and who is able to support young people and families and their colleagues whilst delivering a excellent standard of care; in what sometimes can be highly complex and challenging circumstances.
Working within a young person eating disorders service is incredibly rewarding and due to the diversity of presentations is a fantastic opportunity for further development of skills and knowledge.
Main duties of the job
The post holder will be required to complete initial assessments alongside other colleagues.
Hold a case load of young people and families as a lead practitioner delivering the appropriate evidence based interventions.
Lead practitioner role includes ensuring all aspects of the young person care is kept up to date, risk assessments, care planning and the ongoing review and evaluation of the effectiveness of the care being offered. Supporting transitions to other services, by making timely referrals and attending meetings as necessary.
Liaising with other teams within CAMHS and outside agencies to ensure joined up care and safe management of clinical risk.

This post offers the Trust's £4,000 Incentive for band 5-7 Nurses & Practitioners, as well as a relocation package up to a maximum of £8,000 (subject to terms and conditions).
