Hastings Urgent Care Services have an exciting and new 8a Advanced Clinical Practitioner post available. The successful post holder will work clinically across the Urgent Care Pathway in Hastings, whilst providing leadership and line management to junior colleagues within the integrated multi-professional team. The post holder will be an expert nurse with extensive clinical experience in the management of patients in crisis and the ability to clinically lead a team.We welcome dynamic applicants from adult Mental Health Nursing and AHP backgrounds. This is an exciting opportunity to join and help shape the Urgent Care services in line with Community Transformation and Mental health long term plan.
The Urgent Care Pathway provides care to people in their homes and the Acute General hospital. The Crisis Resolution and Home Treatment team and Mental Health Liaison Team sits within this pathway, and aims to avoid hospital admissions as well as reduce length of inpatient admissions. The overall goal is to enable people to recover quicker in their own homes and live independently for longer. The teams work closely with the Haven (extended assessment clinical area) , Blue light emergency services, primary and secondary mental health services in the local area.
Main duties of the job
Through these teams, older people and adults with complex health needs who urgently need care, can get fast access to a range of health and social care professionals. You will work as a member of the multi-disciplinary team supporting patients who present in crisis utilising your advanced clinical assessment skills and non-medical prescribing qualification to reduce unnecessary admissions to hospital and support admission avoidance strategies. You will also work along side our adult social care colleagues in response to mental health act referrals and assist the exploration of least restrictive treatment options and alternatives to hospital admission.
