We are seeking a Band 7 Learning Disability Nurse to join our Integrated Community Learning Disability Team in Bognor Regis. As a Lead Learning Disability Nurse in our team you will be a key member of our multi-professional health team, working in close partnership with Social Services colleagues to support people who have a learning disability, but who also have complex physical and mental health needs including behaviours that challenge.

Our community learning disability team works across Bognor Regis and surrounding areas to provide specialist community support for adults with a learning disability, their families and carers. Our teams include specialist nurses, occupational therapists, psychologists, psychiatrists, social workers, physios, speech and language therapists and more.

This post offers the Trust's £4000 Incentive for band 5-7 Nurses (pro rata applies and subject to terms and conditions).

Main duties of the job

Within this role you will be supporting the nursing team to provide high-quality assessment, support and advice to people with learning disabilities and their families and carers. You will be responsible for a caseload providing and/or arranging access to specialist focused health assessments and interventions. You will need to monitor identified health care and treatment plans relating to complex issues such as physical health, epilepsy, challenging behaviour and mental health. You will be able to identify and manage risk and offer care plans to support individual needs. You will be confident in advocating on behalf of the person with learning disabilities. A good working knowledge of Adult Safeguarding, the LeDeR programme and Transforming Care agendas will be essential.

We will need you to be flexible with excellent communication and interpersonal skills. You will need to have good organisational abilities to manage your diary and own case load. You will need to have good IT skills to use various computer programmes and be able to work remotely if needed. You will need to be able to travel around West Sussex and sometimes wider areas for home visits and meetings.

You will deliver support and development to the nursing team. Providing regular supervision, annual appraisal and consideration of training, mentoring and coaching. You will offer mentoring to student nurses and Trainee Nurse Associates (TNAs). You will also work closely with the Team Lead to support the wider team.

You will be required and supported to continue your professional development in line with your registration, validation and Trust policies and procedures.
