Job summary
Fylde and Wyre Community Mental Health Team has an exciting opportunity for a Band 6 Community Mental Health Nurse to join the team. This post provides the opportunity for a forward thinking and enthusiastic individual to develop and implement safe, sound and supportive services that embrace the provision of holistic care. With a strong emphasis on multidisciplinary working and the principles laid out in the Recovery Model, you will need to be an innovative thinker and be able to form effective relationships.
Main duties of the job
You will have a working knowledge of assessments, care planning and the risk assessment and management of people under our community services, and the ability to explore issues relating to the impact of health and social needs on mental health and well-being.Fylde and Wyre CMHT works over 2 bases due to the geographical size of the catchment area. You will have a main team base but there is the expectation of support across both sites as required.Fylde and Wyre CMHT celebrates our staff achievements and wellbeing, with regular team sessions and receiving feedback from our patients, families and carers.Contact details
