An exciting opportunity has arisen for a Nursing Associate Practitioner to join the Adult Community Learning Disability Team in Fylde and Wyre.The Learning Disability Service strives to provide forward-thinking high-quality services, working in partnership with other agencies. In addition to working with adults with learning disabilities and autism, we work closely with their families, and we have a strong commitment to health promotion through service development and training initiatives. We are committed to helping our staff develop and you will be part of an enthusiastic and experienced team.

The post holder will work within an integrated Multi Disciplinary Team whilst supporting the nursing workforce. The Nurse Associate role includes undertaking home visits and having a presence within the communities for people with a Learning Disability, their families and carers.

Main duties of the job

The role will comprise of gathering holistic health information which supports the nurses in completing a detailed health assessment, then with oversight of the nurses following through with all health actions identified.

Supporting nursing staff with specialist Learning Disability assessments, completion of core documentation and identifying clinical risks, including safeguarding concerns. Helping with the promotion of reducing health inequalities and premature deaths of individuals with a Learning Disability.

The role will also support the nurses with clinical support to adults with Learning Disabilities who have additional complex needs, which may include challenging behaviour, physical and sensory impairment, mental health and forensic issues. The post holder will play an integral role in the delivery of care within these pathways within a variety of community settings.

We are looking for a Nursing Associate Practitioner with an up-to-date knowledge of current health legislation effecting people with Learning Disabilities including knowledge of the Transforming Care agenda. They need to be adaptable, have good communication skills and self-motivation is essential. The successful applicant will have proven skills in working with people who have a learning Disability, complex health needs and challenging behaviour. They will also be able to demonstrate that their practice is evidence based and innovative.
