Job summary

The Lancaster & Morecambe Older Adults Community Mental Health Team (CMHT OA) are looking to recruit a Band 5 Community Mental Health Nurse. This is a permanent, full-time position of 37.5 hours. The team is based at DeVitre House in Lancaster and covers the Lancaster & Morecambe district. The team operates between Monday - Friday, 09:00hrs - 17:00hrs.

We are looking for a motivated and enthusiastic RMN who is driven to provide excellent mental health services to the local older adult population. The post-holder will demonstrate compassionate caring behaviours, have excellent communication skills, be reliable and work well within a team. You will be able to work calmly and efficiently whilst being adaptable to changing situations, remaining focused on providing a person-centred approach to care and treatment.

Main duties of the job

Supports Band 6 practitioners with their caseloads in capacity as 'associate practitioner,' supporting care and treatment pathway, and discharge planning.

Provides assessment, advice and treatment to people with Dementia and complex mental health needs, alongside B6 practitioner.

Delivers interventions in accordance with care pathways for given specialism, under the supervision of senior clinical staff.

Undertake physical health observation appointments as directed and stipulated by the MDT.

Works closely with other professionals within and outside the Community Team.

Actively supports the Team Leader in the achievement of KPI / CQUIN and related County Council targets, and maintenance of registration with regulating bodies.

Support the team in achieving specific objectives as directed by the Team Leader.

Provides direction to support staff within community services.

Supports a culture of ongoing learning and development on an individual and team basis to ensure high standards are maintained and, where appropriate, contribute to the delivery of training as appropriate.

Provide leadership to junior colleagues, i.e. acting as a positive role model

The post holder will have a positive attitude to the care of the service users, be adaptable and resourceful, and able to lone-work under specific circumstances.
