Job summary
Fylde and Wyre Community Mental Health Team has an exciting opportunity for a Band 6 Community Mental Health Practitioner to join the team. This post provides the opportunity for a forward thinking and enthusiastic individual to develop and implement safe, sound and supportive services that embrace the provision of holistic care. With a strong emphasis on multidisciplinary working and the principles laid out in the Recovery Model, you will need to be an innovative thinker and be able to form effective relationships.
You will have a working knowledge of assessments, care planning and the risk assessment and management of people under our community services, and the ability to explore issues relating to the impact of health and social needs on mental health and well-being.
Fylde and Wyre CMHT works over 2 bases due to the geographical size of the catchment area. You will have a main team base but there is the expectation of support across both sites as required.
Main duties of the job
o To actively promote teamwork, respect, integrity, accountability, excellence and compassion within the clinical team in accordance with the Trust values and code of conduct.o To undertake mental health assessments including Risk assessments within a multi-disciplinary assessment team. o To be proficient in referring to other clinicians within the service or referring to other services/agencies where necessary. o To provide a fast, responsive service to assess individual's needs.o Providing a communication structure between GP's, IAPT and non IAPT services in LSCFT, non-statuary service, as a means of ensuring the highest possible standards of patient care.
