Job summary
This is an exciting opportunity to work within an innovative, dynamic and growing multi-disciplinary Community CYPMH in LSCFT- Fylde and Wyre. The children & young people who access our services present with a wide range of emotional and mental health difficulties ranging from moderate to severe.
You would be joining an established, supportive and hardworking team who are passionate about improving the outcomes for the children, young people and their families/carers.
As a Community CAMHS Mental Health assistant Practitioner you will be an integral member of the multidisciplinary team offering consultation, assessment and evidence-based interventions to children and young people we serve.
Main duties of the job
Main duties of the Community CAMHS band 5 Mental Health Practitioner include:
Therapeutic interventions , assessment and triage
Support to Conduct initial assessments to determine appropriate care pathways. Develop skills and knowledge to complete assessments autonomously
Manage and escalate risk in line with safeguarding and clinical protocols
Provide short-term therapeutic interventions for young people awaiting longer-term support
Building therapeutic relationships with children and young people and delivering evidence based interventions (including group treatment)
Signpost and liaise with internal teams and external agencies to ensure timely access to services
Maintain accurate documentation and comply with national data timescales
Work collaboratively within a multi-disciplinary team to support continuity of care
Attend daily screening and referral meetings to support with signposting and completion of screening notes
Contributing to all aspects of positive team functioning
Contributing to ongoing service innovations and development
You are encouraged to enrol for the DBS Update Service. An annual fee of £16 per year applies
