We are seeking a dedicated and compassionate Mental Health Practitioner to join our Community Mental Health Team based at Pathways MHRC in Bamber Bridge.

As a Band 6 Keyworker, you will hold a defined caseload and deliver holistic, recovery-focused care to adults with complex and enduring mental health needs. You will play an active role within a multidisciplinary team (MDT), contributing to assessments, care planning, risk management, and evidence-based interventions.

A thorough understanding of psychotropic medications, including Clozapine and Lithium, as you will be responsible for supporting safe medication management and monitoring within community settings.

Main duties of the job

Act as keyworker for a defined caseload, delivering assessment, care planning, implementation, and review under the Care Programme Approach (CPA).

Work collaboratively within a multidisciplinary team, contributing to shared decision-making and holistic care.

Support service users prescribed psychotropic medications, including Clozapine and Lithium, in line with Trust policy and national safety standards.

Undertake comprehensive risk assessments and develop collaborative management plans to promote safety and recovery.

Deliver psychosocial interventions to promote independence, wellbeing, and recovery.

Build and maintain therapeutic relationships with service users, carers, and families, embedding the principles of the Triangle of Care.

Liaise effectively with partner agencies, primary care, and voluntary sector organisations to ensure coordinated, person-centred care.

Contribute to a positive, learning culture, promoting reflection, innovation, and adherence to professional and Trust values.
