Children's Community Learning Disability Nurse

Job summary
We are seeking a highly motivated and compassionate Registered Nurse to join our experienced Children's Community Learning Disability Team.
You will work with children and young people aged 0-18 who have a diagnosed learning disability and may present with additional needs such as:
- Autism spectrum conditions
- Communication difficulties
- Challenging behaviours
- Sensory processing differences
- Mental health concerns
- Physical health needs
You will provide specialist nursing input across a range of community settings including homes, schools, respite services, and clinics. The role involves family-centred care, working closely with parents/carers, education providers, social care, and other health professionals to promote positive outcomes and reduce health inequalities
Main duties of the job
The key responsibilities of the job include:
- Undertaking holistic assessments and contribute to multi-agency care planning
- Deliver evidence-based interventions to support emotional wellbeing, behaviour, and physical health
- Provide specialist advice and training to families, schools, and other professionals
- Facilitating and conducting workshops and group work
- Support safeguarding processes and contribute to risk assessments
- Promote health education and preventative care
- Contribute to service development and quality improvement initiatives
- Maintain accurate clinical records using electronic systems (Rio)
