Job summary

Burnley Community Mental Health Team are seeking to appoint a Band 5 Mental Health and Wellbeing Practitioner. This post provides the opportunity for a forward thinking and experienced individual to develop and implement safe, sound and supportive services that embrace the provision of holistic care. With a strong emphasis on multidisciplinary working and the principles laid out in the Recovery Model, you will need to be an innovative thinker and be able to form effective relationships.

You must be a registered mental health and well being practitioner, with excellent leadership and clinical skills. You will also need to have a working knowledge of the care programme approach, risk assessment and management, and the ability to explore issues relating to the impact of health and social needs on mental health and well-being.

Main duties of the job

To work alongside service users, families, carers and multi-disciplinary team members to co-ordinate care as a key worker, supporting collaborative decision-making about care and treatment.

To deliver a set of wellbeing-focused psychologically-informed interventions, aligned to cognitive-behavioural principles, based on the best evidence available, that address problems often experienced by people with severe mental health problems.

Specialist provision comprises inpatient child and adolescent mental health services, perinatal mental health and forensic services including low and medium secure care.

  • oRegistered as a Mental Health and Wellbeing Practitioner on a professional register recognised for this role by NHS England.
  • oA graduate certificate (Level 6) or postgraduate certificate (Level 7) delivering the national curriculum for Mental Health and Wellbeing Practitioners (Adult Specialist Mental Health).

Knowledge

Essential

  • oAble to plan and deliver interventions with a variety of service users presenting with mental health difficulties, their families and carers and the clinical team, on a daily basis.
  • oAble to establish and maintain empathic, supportive relationships with people in significant distress or who are cognitively impaired, their families and carers
  • oAble to receive, understand and communicate confidential client information of a sensitive and often complex nature, including discussing care with family members within boundaries of confidentiality.
  • oAbility to work successfully in a team
  • oAble to prioritise workload according to peoples' changing needs and the priorities of the team

Experience

  • oExperience of working with people with mental health needs gained through a graduate certificate or postgraduate certificate training with significant supervised practice.
