37.5 Hours over a 7 day working week covering 8am-8pm

Contract: Fixed term until 31/3/26

We are seeking a Triage Practitioner to join our Care Connexions Team, playing a vital role in providing timely support to people experiencing health or social care crises in their own homes.As a Triage Practitioner, you will be the first point of contact for patients, families, carers, and healthcare professionals seeking urgent support. You'll assess the nature of the call, gather key clinical information, and ensure appropriate triage and dispatch to the appropriate team, with the support of a clinical co-ordinator.o Answer and triage incoming calls efficiently and empathetically.o Identify urgent needs and prioritize responses based on clinical risk.o Liaise with nurses, therapists, paramedics, GPs, and other stakeholders.o Maintain accurate and timely records in line with protocols.o Support coordination of urgent community response visits within 2-hour timeframes.o Experience in a call-handling or healthcare setting (essential).o Strong communication and decision-making skills.o Ability to remain calm and professional in high-pressure situations.o IT proficiency and confidence using electronic record systems.o Knowledge of urgent care pathways or community health services

Main duties of the job

The post holder will provide administrative tasks & triage support for the Care Connexions Team. The role will include integration across Lancashire and South Cumbria. This will involve working across organisations to provide admin support and be the first point of contact of communication with patients, patients' relatives, healthcare professionals and other callers, responsible for the receipt, allocation and distribution of all referrals to designated services. Using set procedures and protocols the post holder will electronically record patients' demographics and basic clinical information which may include documenting readiness to change, screening for red flags and ensure they are directed to the most appropriate healthcare professional or end support. This role will be flexible and the post holder will be expected to work at Leyland House as part of the Care Connexions triage hub.

The post holder will provide a basic assessment as directed by clinicians, basic advice including how to manage their symptoms at home and health information, supporting them to access the appropriate level of care.

There will also be a requirement to ensure current documentation for the integrated records are in place and ready to use by clinicians. The post holder will be expected to support and coordinate staff within the service.
