Job summary

The Fylde Coast Community Mental Health Team is an Older Adults Community Mental Health service based at Shorelands Blackpool. We are a 5 day a week service with operating hours from 9am to 5pm. We are a multi- disciplinary service, with close working links with other health departments, social services and third sector/voluntary agencies.Our service users present with severe and enduring mental health difficulties of a functional or organic nature, often combined with social care and physical health needs and may require long term interventions from the CMHT.Please ensure that you read the Person Specification attached below as your application will be measured against this document. Main duties of the job

We are looking to recruit a motivated and dynamic Band 6 CMHN. It is expected that the successful candidate will have a good knowledge base of organic/functional mental health illnesses and the impact of these have on our older adult community. Where indicated they will be able assess and apply interventions in response to service users ongoing and changing mental health and or physical health needs, co-ordinating the input of other professionals as appropriate and evaluating and monitoring risk factors on a continual basis. There will be an expectation that the CMHN will work autonomously under the supervision of a senior Band 6 Nurse/Deputy and will care coordinate own case load.After a period of induction / preceptorship, involvement in the team's development is integral to this role via supervision of other staff, mentoring students, equally we promote professional development via internal / external training to meet professional and revalidation requirements.Please note that further communication from Lancashire & South Cumbria NHS Foundation Trust (LCFT) will normally be to the email address given on the application form.
