Job summary

We are looking to recruit a Band 5 Community Mental Health Nurse who has an enthusiasm for and passion to work within older adult's mental health services.

The Band 5 Community Mental Health Nurse will have the opportunity to consolidate their skills in mental health nursing and we want to recruit someone who is keen to develop the specialist skills needed in an older adults mental health team.

This post does offer a preceptorship so newly qualified nurses are welcome to apply.

Fylde Coast RITT is an Older Adults Community Mental Health service based at the Lytham Unit, Lytham hospital.

We are a 7 day a week service with operating hours from 8am to 8pm, shifts worked at present can be Early (8-4pm) Late (12-8pm) including some weekend work (at present one in four)

Main duties of the job

- To undertake mental health assessments including risk assessments within a multi-disciplinaryassessment.- Working closely with the Consultant Psychiatrist, thus ensuring that there is a multidisciplinary team approach.- To be proficient in referring to other clinicians within the service or referring to other services/agencies where necessary.- To provide a fast, responsive service to assess individuals needs for access into mental health services.- Providing a communication structure between GP's , IAPT and non IAPT services in LCFT, non-statuaryservice as a means of ensuring the highest possible standards of patient care .- To provide mentoring and clinical supervision to other staff and trainees on placement.
