Job summary
A great opportunity has arisen for a highly motivated, suitably qualified, confident and forward thinking individual to work as a healthcare assistant. This will be a community based role supporting band 6 MH practitioners in the development and implementation of a community care plans. You will be expected to work independently in the community and demonstrate good IT skill when inputting data. Good communication skills and de-escalation skills are vital in this role.
The Frequent attenders team is situated in Lancaster, and is supported by the frequent attender's team in Barrow.
Main duties of the job
This is a small team and you will be expected to work closely with two band 6 practitioners, they are responsible for the implementations of community care plans. The role of the team is to support, build therapeutic relationship thus reducing risks for individuals who may be placing themselves in difficult situation. Our aim is to support individuals in the reduction and frequency of emergency care use, and the reduction of self-harm. The team works very closely with other areas of the trust to support each other and there is a clear focus on support and professional development of our nurses and support workers.
We have a wealth of disciplines based around our team including Psychologists, Occupational Health Nurses, Health and Wellbeing Workers, Physical Health Nurses, Pharmacists, and Psychiatric consultants. We have also developed good networks with the police, NWAS, Morecambe Bay trust and substance misuse services. This ensures a holistic way of working and enables us to deliver the highest standard of care to our patients.
The successful post holder may in times of need be expected to rotate across the specialty areas as and when needed, but primarily they will work within the FA team 9 to 5 over three day (to be agreed)
