An exciting opportunity has arisen for Band 6 Qualified Learning Disability Nurse to join the innovative Discharge Co-ordination team.
There has never been a more exciting time to join our expanding Learning Disability Services and these posts will allow you to use your clinical and leadership skills to make a real difference.
Main duties of the job
Working closely with partner agencies, such as the ICB, Local Authority and colleagues in inpatient settings; these new and exciting nursing roles will co-ordinate safe and effective repatriation of people in out of area placements back to Lancashire and South Cumbria.
The Discharge Co-ordination Service support Learning Disability patients who are in out of area providers across the North West and, in some cases, nationally (the team has access to a Trust pool car for longer journeys and the Trust have recently announced increased mileage payments for those exceeding the 3,500 mile limit). Travel outside of Lancashire and South Cumbria will be a feature of the role possibly overnight stays if required. The team now also support discharge pathways for patients in local beds.
