Job summary

PENNINE HOME TREATMENT TEAM

Based Daisy field Mill Blackburn and Pendle House Nelson

Mental Health Practitioner (Band 6) 37.5 hrs per week, including nights and unsocial hours as per rota requirements.

Pennine Home Treatment Teams are split into 2 locality bases covering the Pennine Area. We have an exciting opportunity for a forward thinking, highly motivated, innovative, dedicated practitioner to provide care and treatment for those service users who require support from the Home Based Treatment Team.

Practitioners in the team are dedicated to working with vulnerable service users in their own homes, preventing further deterioration to avoid the need for admission to hospital. We have close working relationships with Community Mental Health Teams and other services across the network such as Mental Health Liaison, Frequent Attenders and Early Intervention teams. We work closely with the Initial Response services where you will also be expected to be located at times to support the pathway.

Main duties of the job

You will have experience of working with service users who present with complex mental health needs.

You should be able to demonstrate evidence of care that facilitates a positive culture of patient engagement, a commitment to enhancing the service user and carer experience and of on-going professional development.

You must have the ability to positively influence and adapt to a rapidly developing service that is able to meet the needs of mental health services in Lancashire.

You will have experience of implementing effective, evidence based care management / assessment skills and display excellent communication skills.

The post holder will be able to work independently without close supervision as well as being committed to a Multi-Disciplinary team Approach (MDT) are essential requirements for the post.

You will be supported to develop within your role and reach your aspirations with a supportive team, committed to the wellbeing of our staff.
