Job summary
This is an exciting opportunity to join the Out of Area flow team for Lancashire and South Cumbria NHS foundation trust as a B6 Out Of Area Flow Practitioner.
The OAPs team have responsibility for having oversight of all Lancashire and South Cumbria residents who are placed in an out of area bed.
The Trust has several contracts with Independent sector Mental Health Acute and PICU beds providers and the team has clinical oversight of the patients in these beds.
The team works closely with the Bed Hub and OAPs providers, monitoring patients in out of area beds and support with safe and timely discharge.
This is an interesting development opportunity for experienced practitioners
Main duties of the job
- The post holder will provide links with the Bed Management Team and Flow Leads to ensure clear and precise communication with internal and external services.
- The post holder will be support the Flow and Capacity Team and the Commissioning and Integration team to provide confident experienced clinical input to the Out of Placements (OAP's), service users currently admitted to inpatient services outside of Lancashire Care.
- The post holder will act as an empowered clinical leader to ensure the patients who are identified for discharge and/or further enhanced community support have the most appropriate setting to enhance their recovery in a timely manner.
- The post holder will monitor quality across all Out of Area placements ( OAP's) commissioned by the trust
- The practitioner will be expected to travel via car to inpatient settings outside of Lancashire Care to review service users and discuss flow and capacity with private providers.
- The post holder will be expected to work within a fast moving environment.
- Ability to explain complex processes and clinical information to patients and their carers or representatives in a way that they are understood. To actively promote, where possible, the recovery model for mental illness thus inspiring hope to the individual and their families/carers.
