Job summary
An exciting opportunity has arisen for a forward thinking, highly motivated, innovative and experienced registered Mental Health Practitioner to provide team Clinical leadership to L&M HBTT. You will be responsible for providing clinical leadership and ensuring that the care provided is safe, effective and supports service users and carers.
You will be able to reflect LSCFT values of being kind, respectful, working as part of a team and always learning. You should have experience of working as a Band 6 or current band 7 and demonstrate evidence of learning within teams and supporting individuals through change, including facilitating a drive for continued staff development.
Experience of working within the community Urgent Care pathway is desirable. You should also be able to show a commitment to supporting engagement with service users and carers to support service development promoting a positive culture of both patient and carer / family engagement.
You should be able to demonstrate effective, evidence based care management, risk assessment and leadership skills. You must have the ability to positively influence and adapt to a rapidly developing service that is able to meet the needs service users and carers.
You will provide specialist clinical input to the team and carry a small clinical caseload of complex cases, demonstrating competence in complex decision making, assessment and the management of clinical needs within the specialist area of practice.
Main duties of the job
The post holder will:
Be responsible and accountable for ensuring the delivery of safe, effective and evidence-based care to Service Users as allocated within the designated pathway
Lead on the implementation of evidence based clinical protocols/pathways
Embed clinical pathways and support staff development in relation to this
Contribute towards the continuous development of the clinical pathways within the service promoting a learning culture within the team
Clinically support the flow of patient activity and bed availability by providing advice and support to colleagues in the Bed Hub in relation to relevant service user group
Support the service in its implementation of clinical effectiveness programs by taking part in any agreed research, quality improvement projects and audit projects, leading on key aspects when identified as appropriate
Provide clinical leadership in relation to area of specialism using the most up to date and evidence based theory, and contribute towards service development, quality governance and assurance
Offer expert clinical advice for service users, carers, staff and other professionals
Make a contribution to training, teaching and supervision of colleagues including ensuring delivery of high quality care and a good understanding, knowledge, systems and processes across the Team
Work in partnership with service users, carers and other agencies to improve patient outcomes and experience
