Job summary
Are you a registered Mental Health Nurse looking for a new challenge and an opportunity to develop in your role?
We are seeking to recruit enthusiastic & committed Band 5 RMN's for the Preston West Community Mental Health Team. You will be part of a dynamic and supportive multi-disciplinary team, and be involved in work on the team duty system in order to support adults with a variety of mental health issues.
You will be a Registered Mental Health Practitioner, ideally with previous experience of working in community mental health services and completed your preceptorship. The role will involve assessing and formulating individual intervention plans and outcome measures for adults with mental health issues and includes supporting families and carers.
You will need to have up to date knowledge of NHS policies and practices in mental health, include NHS England changes around the lead professional role and new Government changes. You should be enthusiastic around the work and care you deliver to support people with a wide range of mental health issues and have a good understanding of the processes around primary and secondary care services.
Main duties of the job
Your role will primarily involve working for the CMHT at West Preston, West Strand House, Preston, Monday to Friday 0900hrs-1700hrs. The role will provide a consistent knowledge and expertise within the team for decision-making and service user's queries.
- Work as a member of the multi-disciplinary/agency, providing a nursing contribution to the teams functioning.
- Completion of CMHT assessment documentation, risk assessment, and liaison with the service user, and any ongoing agencies involved or appropriate signposting to outside agencies, including VFCSE, dependant on the individuals needs.
- Administration and monitoring of medication to facilitate compliance and adherence to treatment. Provide treatment through deep intra-muscular injection for service users attending the depot clinic or requiring depot administration in their home. Along with clozapine/ Lithium monitoring where required.
