Job summary

This is an exciting and new opportunity for a Healthcare Assistant to join the Fylde Frequent Attenders Team.

Service Users within the "Frequent Attender Programme" are high intensity/complex individuals with multiple issues and needs. They can be frequent attenders within the Emergency Department or high users of Section 136.

To work assertively with people within the Locality to enhance their engagement with multi agency services to support them within their recovery. This will involve working collaboratively with other agencies to aid engagement to meet the needs of the service user

To positively promote the independent living of Service Users within the community, to facilitate social and leisure activities for Service Users, and to focus on their direct needs, working across boundaries of care to promote social inclusion, independence and maximise the effectiveness of STR services within available resources.

To assist in the provision of quality evidence based practice, providing advice; support and care under the guidance of the Frequent Attenders Case Manager for Service Users and their carers within multidisciplinary setting in a defined catchment area/locality.

The team operates Monday to Friday 9-5.

Main duties of the job

The post holder is responsible for ensuring accurate and timely written/electronic records are kept which comply with the Trust, reporting on any issues as appropriate.

The post holder is responsible for ensuring that they clearly communicate with Service User's and Carer's, actively listen to establish an understanding of their needs, enabling them to make informed decisions about their care.

Ensure a friendly and helpful attitude to patients, visitors and colleagues, at all times.

Interacting with Service Users which may involve providing information and using skills to overcome barriers in an empathic and re-assuring manner

  • Significant experience in mental health care or relevant NVQ level 3 qualification.
  • Good level of education. Able to demonstrate competence in written and oral English.
