An exciting opportunity has arisen for Band 6 Qualified Practitioner (Nurse, AHP, or Social Worker) to join our award winning Specialist Autism Team as an innovative Discharge Coordinator.

Main duties of the job

Working as part of an MDT and closely with partner agencies, such as Local Authority Social Workers, these new and exciting discharge coordination roles will coordinate safe and effective repatriation of people in acute mental health inpatient beds back to their local community.

The Discharge Co-ordination role will support autistic patients who are in an LSCFT inpatient setting and in some cases out of area providers across the North West. Travel across Lancashire and South Cumbria will be a feature of the role.

About us

The Specialist Autism Team is an innovative and award winning team that came into being 2 years ago after investment from the Integrated Care Board. The Team provides three functions:

Outreach: working to prevent avoidable admission of autistic people to acute MH beds, and improving their health and lives.

Inreach: working with our LSCFT inpatient colleagues to improve the efficacy of inpatient stays for autistic patients, making their care as person centred and autism-informed as possible to achieve good health outcome and reduce their length of admission.

Discharge Coordination: working to ensure that discharges of autistic patients from acute MH beds after timely, well planned and sustainable. This requires multi-disciplinary and multi-agency working at system level to ensure that delays to discharge are minimised and community placements / arrangements are identified that can successfully support people to remain in the community and live fulfilling lives.

Lancashire & South Cumbria NHS Foundation Trust provides a range of services including secondary mental health care across this area. Specialist provision comprises inpatient child and adolescent mental health services, perinatal mental health and forensic services including low and medium secure care.

The Trust also provides a range of physical health and well-being services in the community alongside a range of partners in the Lancashire, Cumbria and Sefton area.

  • Relevant Core Professional Qualification and registered with the appropriate professional body (NMC or HCPC registered) 2 -3 years' post-registration experience

  • Teaching/Mentorship post-registration qualification
