Job summary
An exciting new opportunity has arisen for a Technical Instructor/Assistant Practitioner to join our expanding virtual ward / urgent response team based at Leyland House.
This will be working alongside a multi professional team of therapists, advanced clinical practitioners, nursing colleagues and social care staff to support and keep patients safe in their own home across Chorley, Preston and South Ribble area.
Virtual wards are a national initiative defined as:
'A virtual ward is a safe and efficient alternative to NHS bedded care and allow patients to receive care, remote monitoring and treatment they need in their own home, safely and conveniently rather than in hospital'
We are looking for an innovative, flexible and motivated Technical Instructor/ Assistant Practitioner who is able to work across a range of settings and preferably has experience of Occupational Therapy and Physiotherapy interventions within a rapid assessment and / or admission avoidance services.
We are looking for a Technical Instructor/ Assistant Practitioner with excellent communication skills and who has the ability to work autonomously as well as part of a team.
Main duties of the job
To provide a high standard of clinical care for individuals, under the supervision of the registered practitioner, and acting within the standards, protocols and guidelines of the rehabilitation service.
To assist in the assessment and therapeutic management of patients' health and well-being needs.
This role involves working with patients in their usual place of residence, under the direction of clinical therapists within the team, to undertake non-complex assessments, equipment provision and carrying out treatment plans where appropriate.
The service will operate 365 days a year, 8.00am - 8.00pm.
This post is subject to an enhanced DBS Disclosure and the successful applicant will be required to pay for their DBS check.
