Job summary

Preston, Chorley and South Ribble, Rapid Intervention and Treatment Team (RITT) are seeking an experienced Band 6 Registered Mental Health Practitioner to join the Home treatment function of the team. This is an exciting opportunity and the successful candidate will be part of an evolving service that has the needs of the older adult population at its heart. We are looking for someone who is enthusiastic and passionate about older adults to come and join a forward thinking, dynamic and constantly developing team. Home treatment work to support patients in the community to prevent hospital admission and facilitate early discharge.

The team provides high quality care working with older adults with dementia or significant functional / psychological mental health needs, ensuring they have access to timely, intensive and specialist mental health care. We also work with families and carer's and alongside other agencies to promote well-being and support for individuals and their carer's / families.

Main duties of the job

RITT consists of 2 functions including, Home Treatment and Care Home Liaison. Although this post is for the Home treatment team function the post holder would be expected at times to cover a 7-day service, within the hours of 8am - 8pm across the functions. The applicant must have experience of working with older adults with dementia and be able to demonstrate a working knowledge of older adult mental health and the needs of individuals in this group.

You will have excellent clinical skills along with outstanding communication skills. You will also need to have a working knowledge of the care programme approach, assessing risk and developing robust risk management plans.
