An opportunity has arisen with the service, for a Band 3 Health Care Assistant post to work within the Specialist Community Learning Disability Team covering the Morecambe Bay area and therefore applicants must be able to travel independently of public transport. We are looking for a highly motivated and a conscientious team player who has excellent communication and clinical skills, looking to build experience in working with community teams.
The Learning Disability Service strives to provide forward thinking high quality services, working in partnership with other agencies. In addition to working with adults with learning disabilities and autism, we work closely with their families, and we have a strong commitment to health promotion through service development and training initiatives. We are committed to helping our staff develop and you will be part of an enthusiastic and experienced team.
Main duties of the job
The post holder will work within an integrated health team, providing support to adults with learning disabilities who have additional complex needs, which may include challenging behaviour, physical and sensory impairment, mental health and forensic issues. The post holder will play an integral role in delivery of care within these pathways within a variety of community settings.
As a band 3 Health Care Assistant, you will be expected to support the clinical activity of the teams in meeting the needs of local people with learning disabilities in the community. You will have skills in working with adults with learning disabilities, and associated communication and behavioural needs. The post holder will undertake delegated duties by senior nurses, and will be accountable to the Operational Lead and Senior Community Nurse - Locality Lead Nurse, and will be providing support to the clinical team within the integrated health team, to ensure effective and timely clinical outcomes are met.
It is expected you will be able to use IT clinical record systems.
