Job summary

The team of Health Care Assistants and Nurses provides advice, assessment, and treatment for patients experiencing bladder and bowel dysfunction .

Assessments are carried out in clinic settings, patients' own homes, or within nursing and residential care environments.

In addition to direct patient care, the team offers advice, support, and education to other healthcare professionals, including care home staff, to manage patients with complex needs effectively.

The postholder will work as part of a multidisciplinary team, assisting and supporting clinical colleagues to deliver high-quality care. This may involve working within patients' homes and other community-based clinical settings.

The role will contribute to:

  • Ensuring safe and effective care for patients.
  • Supporting the delivery of the quality strategy within the designated area.
  • Establishing systems and processes that uphold service standards.

Main duties of the job

Work within established guidelines to identify and manage risk, reporting concerns to the Team Leader.

Ensure delivery of the quality strategy within the designated area, incorporating systems and processes to maintain high standards of care.

Work as an integral member of the multidisciplinary team.

Undertake a range of patient interventions within the scope of NVQ Level 3 skills, as delegated by the Team Leader or qualified clinical staff.

Provide advice to patients and carers on health issues, including health promotion, lifestyle advice, and self-care.

Plan and organise own workload following delegation from the Team Leader.

Adhere to clinical guidelines, policies, and infection control procedures.

Offer advice and emotional support to patients and families in a variety of situations.

Ensure safe moving and handling techniques, including the correct use of equipment.

Communicate relevant information to the multidisciplinary team and other agencies as appropriate.

Undertake ordering and receipt of stores.

Participate in further training to develop skills in additional areas.

Utilise NVQ Level 3 skills within identified areas as required by the clinical team.

Deliver patient care in line with Essence of Care standards and Trust policies.

Document care delivered in handwritten or electronic patient records.

Participate in own PDR with the Team Leader to establish objectives, supervision, and ongoing development.

Comply with all organisational policies and procedures.
