Job summary

An exciting opportunity has arisen for an enthusiastic and motivated Band 6 Occupational Therapist to join Community Occupational Therapy Adult Rehabilitation Team (COTART) / Specialist Falls Team. The successful candidate will undertake comprehensive assessments of service users with a variety of physical, cognitive and/or learning disabilities and will have responsibility for the delivery of Occupational Therapy interventions to patients based in the community, optimising their function within the home environment and helping to prevent hospital admission. Many service users present with complex multi-pathologies which require a high level of understanding of the impact of these conditions/disabilities.

The skills required for this post include practical problem solving skills, with sound clinical reasoning skills. An effective team player; and have excellent organisational and communication skills required to work in a busy service.

You will have access to internal leadership training opportunities, B6 peer support, clinical supervision and CPD opportunities.

Main duties of the job

The post holder will be expected to effectively manage a caseload of patient's co-morbidities as an autonomous practitioner, using evidence based/ patient centred principles to assess, plan, implement and evaluate interventions of the patient group. The post holder will provide leadership for junior staff, when appropriate, through supervision and appraisal. The post holder will participate in planning, development and evaluation of OT services within a designated area/team, holding responsibility for defined projects.

The post holder will take a holistic approach in the assessment and treatment of service users / patients, being responsible for the provision of appropriate, high quality care to a defined caseload which may be of a complex nature, using the appropriate model of care and evidenced based practice.

Work in partnership with other professional to enable patients / clients to be maintained in an appropriate environment, thus avoiding hospital admission.

To contribute to the maintenance and development of The Occupational Therapy profession within the Community Occupational Therapy Service and will facilitate effective learning within the area of practice for all students and practitioners.
