Please note that this is a community based post which will require the successful post holder to travel regularly between multiple sites in order to perform duties.
Kirkby Community Mental Health team are looking to recruit a Band 5 Nurse Practitioner on a permanent contract. We are a supportive, well-resourced and dedicated team and there will be opportunities for professional development within the role.
We are looking for a Registered Mental Health Nurse, to work alongside our large multidisciplinary team. A special interest in risk management/crisis intervention work would suit the applicant and a full induction and training package will be provided to the right candidate.
This is a wonderful opportunity for someone who is interested in developing as a practitioner and person. Development opportunities will be available and regular clinical supervision and support will be provided.
Main duties of the job
The post holder will contribute to maintaining overall standards, which will include assessment of need and delivery of care within the context of Effective Care Coordination.
The post holder will work within an integrated team to ensure continuity of care for service users on their journey through the acute service. The post holder will work from within the community environment but be expected to be flexible in terms of delivering care within a variety of settings.
This role will involve regular travel across healthcare organisation sites in the Knowsley, Liverpool and Sefton areas. Therefore, the post holder must be able to meet the travel requirements of this post.
